When you submit refill requests, transfer forms, or consultation inquiries through our website forms (powered by Jotform), we collect:

• Full Name, Phone Number, Email Address, and Home Address.
• Prescription numbers, prescribing doctor information, and current pharmacy details.
• Health insurance card details for prescription processing.
